It is the patient’s responsibility to know if their insurance company requires a prior authorization. If the proper authorization is not obtained by time of service, the patient will be considered self-pay or will have to reschedule their appointment until authorization is obtained.
All co-pays are due at the time of visit. An additional $25 will be charged if you co-pay is not paid at your visit.
As a courtesy, if you have a private insurance we can bill on your behalf.
